| DME MAC Appeals Fax Reopening Line National Government Services, the Jurisdiction B Durable Medical Equipment Medicare Administrative Contractor (DME MAC) is pleased to announce effective Monday, August 10, 2009 suppliers may submit faxed reopening requests to resolve minor errors or omissions. Suppliers should complete the Jurisdiction B DME MAC Request for Clerical Error and Omission Reopening Form and fax the request to (317) 595-4737. The Jurisdiction B DME MAC Request for Clerical Error or Omission Reopening Form can be found on the National Government Services Web site at: http://www.ngsmedicare.com/NGSMedicare/DMEMAC/Resources/Forms/0309_JBD_ReopeningForm.pdf Faxed requests will be accepted Monday through Friday during the hours of 8:00 AM – 4:00 PM ET, any request received after 4:00 PM or on a Saturday, Sunday, federal nonworkday, or legal holiday will be counted in the next business day s workload. Reminders:Suppliers should not fax a request for reopening if the request was previously mailed;Requests for Redeterminations should not be faxed; andSuppliers should not fax routine correspondence or additional documentation Suppliers are also reminded that National Government Services has 60 days to complete requests for reopenings regardless of how they are submitted. |
