Reopening Requests for Modifiers KX, GA, GZ and GY: Reopening requests (telephone or written) to add, change, or remove modifiers KX, GA, GZ, and/or GY are no longer being accepted for dates of receipt on or after December 1, 2009. In order to add, change or remove these modifiers, a redetermination request or a new claim must be submitted.
- If you receive ANSI 16 and ANSI Remark Code M78 please resubmit the claim(s) electronically with the appropriate modifier(s).
- If you receive ANSI 50 and ANSI Remark Code N115 a redetermination request with proper documentation must be sent to the DME MAC Jurisdiction C Appeals Department. Use the Redetermination Request Form located on CGS website at www.cignagovernmentservices.com.
