Jurisdiction B News: Clarification on Signature and Date Stamps

 
 
Clarification on Signature and Date Stamps National Government Services, the Jurisdiction B Durable Medical Equipment Administrative Contractor (DME MAC) has received several questions regarding a recently released Medicare Learning Network (MLN) Matters article.  One of the highest Comprehensive Error Rate Testing (CERT) errors continues to involve multiple issues related to physician orders, including the lack of a legible identifier.  Suppliers must comply with the requirements that are outlined in MLN Matters article 6261. The article specifically indicated that effective February 2, 2009 signature and date stamps are notacceptable for use on Certificates of Medical Necessity (CMNs) and DME Information Forms (DIFs).  However, please be reminded that signature and date stamps are not acceptable on any medical record documentation.  Some other examples of medical record documentation include but are not limited to detailed written orders, dispensing orders, detailed written orders prior to delivery, and signed and dated statements of compliance from the treating physician. The Program Integrity Manual (PIM), Chapter 3, section 3.4.1.1, states that Medicare requires a legible identifier for services provided/ordered. The method used shall be hand written or an electronic signature (stamp signatures are not acceptable) to sign an order or other medical record documentation for medical review purposes.
