Reminder: DME MAC Appeals Fax Reopening Line

National Government Services, the Jurisdiction B Durable Medical Equipment Medicare Administrative Contractor (DME MAC) now has the availability for suppliers to fax their reopening requests to resolve minor errors or omissions.
National Government Services would like to remind suppliers not to re-fax a request for a reopening if the request was previously mailed or faxed.  Suppliers will receive an acknowledgement letter via mail within 10 business days once the request has been received into our office.  National Government Services has 60 days to complete requests for reopenings regardless of how they are submitted (faxed or mailed).
Suppliers are encouraged to check the status of the reopening or appeal request by contacting the Interactive Voice Response (IVR) system at 1-877-299-7900, and select option 4.  Suppliers should have their, NPI, PTAN (ten digit supplier number), last five digits of the tax identification number (TIN), beneficiary Medicare number, beneficiary first and last name, beneficiary date of birth, and the claim control number (CCN) available when calling. Once the authentication elements have been verified, the IVR will supply the Document Control Number (DCN).  Suppliers can confirm that the DCN noted in the acknowledgement letter is the same as the DCN confirmed via the IVR.  Suppliers may also hear all associated CCNs to the DCN.  In addition suppliers will hear the status of the request, the receipt date, any additional dates of services and the decision.
Faxed requests will be accepted Monday through Friday during the hours of 8:00 AM – 4:00 PM ET, any request received after 4:00 PM or on a Saturday, Sunday, federal non-workday, or legal holiday will be counted in the next business day s workload.
Suppliers should complete the Jurisdiction B DME MAC Request for Clerical Error and Omission Reopening Form and fax the request to (317) 595-4737. Note:
National Government Services would also like to remind suppliers that requests for Redeterminations should not be faxed.  To request a Redetermination, suppliers should submit their requests in writing to the following address:
