| 1099 Form Information for Calendar Year 2008Attention: National Government Services will mail calendar year 2008 1099 forms for all lines of business on January 30, 2009. Providers can expect to receive their 1099 form within 7‒10 business days of the January 30, 2009 mailing date. For calendar year 2008, all Medicare providers and beneficiaries serviced by National Government Services will receive a combined Part A (including home health and hospice), Part B, and Jurisdiction B Durable Medical Equipment Medicare Administrative Contractor (DME MAC) 1099 form from National Government Services.
